Position Purpose: Act as a role model to persons in recovery/seeking recovery; model and mentor the recovery process, and demonstrate healthy coping skills. Help expectant mothers with SUD history to engage in community supports to ensure safe, healthy infants and supported families.

Principal Responsibilities:

Community Case Management

  • Conduct an in-home assessment with the referred parent to identify the full scope of their needs.
  • Facilitates client access to community resources
  • Advocate on behalf of participating mothers to protect the family’s rights and to assist in reducing associated stigma.
  • Assists the client in developing and making contact with natural social support networks.
  • Assist in the development and delivery of a safe care plan for the mother.
  • Maintain contact with the client and/or other relevant individuals or agencies to ensure the achievement of the client's goals and objectives as outlined. This position requires home visitation in addition to other forms of communication.
  • Assess for any new barriers or unmet needs.
  • Work with identified mothers and families, providing regular follow-up from pre-natal care to 12 months post-partum.
  • Document case management services in the client’s file. The services must be linked to the case management.
  • Network with and become familiar with other local agencies and services that might be of service to clients during and after residential placement.
  • Participate in community collaborative groups and the monthly Peer Navigator Mentorship Cohort.

Recovery Coaching

  • Work collaboratively with the other members of the integrated care team (therapists, other recovery coaches, medical staff) to help clients move forward in their recovery.
  • Meet individually with people who would like the assistance and accountability of a peer mentor; conduct “Welcoming Sessions” and develop a Recovery Plan.
  • Facilitate groups that:
  1. Provide support and accountability by discussing struggles, exploring new ways to approach difficult situations, and sharing personal insights.
  2. Teach and strengthen the skills needed to achieve and maintain personal recovery.
  • Introduce and link individuals to community resources, self-help groups, and peer supports outside of the agency to promote community integration.
  • Maintain an environment of welcome and acceptance at the Recovery Center, so that the community values the Center as a place of help and hope. Comfortably interact with all people who stop by the Recovery Center, whether dropping in, attending scheduled programming, or visiting the community.
  • Maintain contact with participants, either by phone or in person, to ensure their ongoing success. Provide re-engagement support as needed.
  • Participate in local team and agency staff meetings.
  • Complete all required documentation in electronic medical records in a timely, appropriate manner.

Preparation, Experience, and Skills:

  • One to three years’ experience as a recovery coach in a licensed SUD program. CCAR/CPRC certification.
  • Three to five consecutive years of quality, active recovery from an addiction. If a former client of Ten16, they must have been out of service for at least 2 years to be considered for a position.
  • Education is a secondary consideration. A preference would be an associate's or bachelor’s degree in human services or a related field.
  • Community Health Worker (CHW) certification within 2 years.
  • Working knowledge of recovery principles, grounded in understandings of multiple pathways, person-centered support, and trauma-informed care.
  • Excellent people skills; experience in outreach, networking, and helping others.
  • Firm professional boundaries; willing to make changes in community interaction, recognizing dual relationships that will be created working as a recovery coach.
  • Strong orientation toward a team approach to service delivery.
  • Demonstrated understanding of confidentiality and good written communication skills
  • Solid computer and time management skills; must be sufficient to meet the documentation requirements and productivity goals of the position.
